On March 24-25, 2011, AMCHP held a Peer-to-Peer Exchange in Salt Lake City, Utah which addressed the topic of medical home for children and youth with autism. State Autism Implementation Grantees from AK, IL, ME, MO, NJ, NM, NY, RI and WI participated in the exchange. The exchange convened a half-day meeting for grantees, and a full-day medical home meeting with more than 100 public health leaders from around the state of Utah.
Objectives
- Recognize elements of a Medical Home and identify how to implement these elements into practice
- Create a vision of Medical Home in the practice
- Identify strategies for building a team to create Medical Home in the practice
- Examine and redefine roles and responsibilities to create a Medical Home team in the practice
- Identify tools, methods and key change concepts that can be applied in the practice for creating Medical Home
- Recognize strategies to implement a family centered approach for children with special needs
