Toolkit to Support Practice Implementation

Welcome to AMCHP's Implementation Toolkits  

This series of toolkits provide Title V programs with NPM specific evidence-based practices, reports, publications, and resources which can be translated (adapted) and implemented at the state/territory and community levels.  Many of the resources found in the toolkits have been identified by reviewing Title V's Block Grant applications on TVIS and following up with those states/jurisdictions which selected each NPM as a priority area.  Other resources have been identified through AMCHP's Innovation Station or have come from partners.  

It is hoped that sharing these resources will encourage other Title V programs to share their NPM specific strategies and resources to support and improve MCH practice implementation across the U.S. An additional aspect of the toolkits is the inclusion of a general implementation guide meant to assist state Title V and CYSHCN leaders and staff in reflecting on progress made towards all strategies being implemented to achieve each of the National Performance Measures (NPMs) as well as identify processes and structures for improving program implementation.    

Using the Toolkits   

To get started, click on the tab of the NPM you are interested in to see an overview of the toolkit structure and its resources. If you're unsure where to start, review the Implementation guide to help you utilize your state action plan to determine which strategies you should continue implementing the way they are being implemented, which strategies you should continue implementing but implement differently, and which strategies you may need to replace with new ones.    

 

 NPM 1: Well Woman Visit

To increase the number of women who have a preventive medical visit.

This toolkit contains examples of strategies state Title V programs can use to address NPM 1. The resources included in the strategic approaches below include programs from AMCHP's Innovation Station and those highlighted in AMCHP's Health for Health for Every Mother: Maternal Health Resource and Planning Guide for States. The strategic approaches include:

  • Strengthen Data Systems and Expand the Evidence Base
  • Enable Healthy Living
  • Improve Access to Care
  • Ensure High Quality Health Care for Women
  • Ensure High Quality Maternity Care

     NPM 6: Developmental Screening

    To increase the number of children who receive a developmental screening. 

    This toolkit has organized NPM 6 specific implementation resources according to the strategies and themes identified in AMCHP's 2017 issue brief entitled: Early Childhood Developmental Screening and Title V: Building Better Systems. These strategies have been organized further into the following strategic approaches:

    • Data Collection, Measurement, and Existing Landscape
    • Policy Research, Development, and Implementation
    • Systems Coordination
    • Technical Assistance and Training
    • Education, Engagement, and Resource Development
    • Other Program Strategies

    The toolkit includes AMCHP resources and state examples that highlight how states are implementing the various strategies (case examples, best practice summaries, toolkits and implementation guides).  Broader resources, including templates, educational materials, and partner resources, can be found on the SPHARC webpage for "Early and Continuous Screening."

     NPM 9: Bullying

    Percent of adolescents, ages 12 through 17, who are bullied or who bully others. 

    This toolkit has organized NPM 9 specific resources according to the change ideas and strategies listed in HRSA/MCHB's Assessing Prevention Capacity & Implementing Change change package.  These strategies have been organized further into the following strategic approaches:

    • Assessment
    • Coalition Building
    • Training and TA
    • Implementation of Evidence-Based Practices
    • Policy and Systems Approaches
    • Communication Efforts

     Implementation Guide

    This guide is meant to assist state Title V and CYSHCN leaders and staff in reflecting on progress made towards strategies implemented to achieve each of the National Performance Measures (NPMs).  

    Reflecting on progress made on an annual basis, such as through the yearly development of the Block Grant application, can help states determine if they are on track for meeting goals and objectives laid out in their state action plans or whether they need to adjust course and potentially explore new strategies to implement.
