Johnson & Johnson Community Health Care Program Invites Applications From Evidence-Based Childhood Obesity Programs

The Johnson & Johnson Community Health Care Program, in partnership with the Johns Hopkins Bloomberg School of Public Health, is accepting applications from nonprofit community healthcare organizations for programs that promote wellness and healthy lifestyle choices to prevent and reduce the impact of obesity in children between the ages of eight and 12. CDC Private Wells/Other Small Drinking-Water Systems Study Funding

This announcement seeks applications from state and local health departments, and U.S. territories and Native American tribal public health agencies. Funding these initiatives will improve the recipients’ ability to address critical gaps in understanding the possible exposures, hazards, and health outcomes in communities and populations using private wells (PW) or small drinking-water systems. Approximately 13 percent of U.S. households, or about 43 million people, use these types of water systems. CDC will fund as many as 15 awards to locate and use data to identify new, or clarify known or suspected, issues related to private wells or small drinking-water systems that may affect public health. CDC Patient-Centered HIV Care Funding

CDC has entered into a public-private partnership with Walgreen Company to develop and implement a model of HIV care that integrates community pharmacists with primary medical providers for patient-centered HIV care. The expected outcomes of the model program are increased retention in HIV care, adherence to HIV medication therapy and viral load suppression. Walgreens has agreed to provide expanded medication therapy management services to 1000 HIV-infected clients, currently or newly initiated on ART, at 10 project sites (~100 persons per site) throughout the country and to work with medical clinic providers to implement the service model. Grantees will work to complete the following activities: 1) develop a patient-centered HIV care model that incorporates Walgreens pharmacists with primary medical providers 2) implement the patient-centered HIV care model and 3) and monitor the performance of the patient-centered HIV care model and adjust the model as needed. 2013 RWJF Roadmaps to Health

Six recognized communities, with the goal of raising awareness and inspiring locally driven change across the country, will receive a $25,000 cash prize and have their success stories shared broadly. Communities will be judged on their accomplishments as well as their capacity for and dedication to creating lasting change by harnessing the collective power of leaders, partners, and community members and for implementing a strategic approach that focuses on the factors that influence health, including health behaviors, clinical care, social and economic factors, and the physical environment. CDC Program to Support New Implementation of State or Territorial Public Health Laboratory Capacity for Newborn Bloodspot Screening of SCID Grant

Newborn screening programs test infants for diseases that require immediate medical treatment but are not apparent at birth. More than 4 million newborns are tested annually in the United States, and thousands of infants and children are saved from disability and death through early detection and treatment. Newborn screening practices, including the types of disorders for which newborns are screened, vary from state to state. The purpose of this funding announcement is to increase the number of states and territories with active screening of severe combined immunodeficiency (SCID) newborn screening programs, and thereby strengthen identification and referral mechanisms to treat a greater number of affected newborns. National Information & Referral Support Center Grant

Administration for Community Living (ACL)/Administration on Aging (AoA) has supported the National Aging Information & Referral Support Center (the Support Center). The Support Center was established to assist the aging network of State Agencies on Aging, Area Agencies on Aging, Tribal organizations, and local aging information and referral (I&R) providers enhance the quality and professionalism of their information and assistance systems. Since 2003, the Support Center has provided technical assistance and training to Aging and Disability Resource Centers (ADRCs). The Support Center provides comprehensive information and technical assistance on a variety of issues relating to information and assistance on aging and more recently programs for people with disabilities. Aging and disability I&R programs and ADRCs continue to evolve and expand their ability to provide person-centered access to information and assistance. I&Rs and ADRCs are working to improve collaboration and coordination to better serve aging and disability populations, families, and caregivers. To assist the aging and disability networks, including ADRCs, to respond to these and other emerging issues ACL will fund a new Cooperative Agreement to provide technical assistance and training. Maternal and Child Health Workforce Development Centers Grant

The purpose of the MCH Workforce Development Centers Program is to support workforce development for State Title V program leaders and staff to meet current public health MCH policy and programmatic imperatives in four key topic areas around implementation of the ACA: (1) access to care; (2) quality improvement; (3) systems integration; and (4) population health management. Up to four (4) national training hubs, or one central hub, based in institutions of higher education, will coordinate training efforts around the key topic areas, to enhance the capacity of the MCH workforce to lead and/or engage in ongoing ACA implementation and public health transformation. Diligent Recruitment of Families for Children in the Foster Care System

This announcement solicits applications to fund cooperative agreements, for the purpose of developing programs to recruit resource families for children in foster care. Applications submitted in response to this announcement should propose projects that will: 1) Implement comprehensive, multi-faceted diligent recruitment programs for resource families, including kinship, foster, concurrent, and adoptive families for children and youth served by public child welfare agencies; effectively identify, engage and support resource families as a means of improving permanency outcomes and building lasting connections for children in foster care. 2) Integrate the diligent recruitment programs into the Child and Family Service Plan and with other agency programs, including foster care case planning and permanency planning processes, to facilitate active concurrent planning activities; 3) Demonstrate the capacity to use this project as a transformative platform for improved system response to permanency and building connections, incorporating changes at the policy and practice level and embracing the philosophy of permanency beginning at entry into the child welfare system; 4) Evaluate the implementation of the comprehensive diligent recruitment programs to document processes and potential linkages between diligent recruitment strategies and improved outcomes; and; 5) Develop identifiable sites that other states/locales seeking to implement improved diligent recruitment methods can look at for guidance, insight and possible replication. Projects to Establish Individual Development Account (IDA) Programs for Refugees Grant

The Office of Refugee Resettlement (ORR) within the Administration for Children and Families (ACF) invites eligible entities to submit competitive grant applications for projects to establish and manage Individual Development Accounts (IDAs) for low-income refugee participants. Eligible refugee participants who enroll in these projects will open and contribute systematically to IDAs for specified Savings Goals, including home ownership, business capitalization, vehicles for educational or work purposes, professional certification, and education (limited to postsecondary education, college entrance exam fees, Test of English as a Foreign Language (TOEFL) and GED preparation and test fees). Advancing aSBI and CHOICES in American Indian and Alaska Native Populations through Training and Technical Assistance Grant

The purpose of this announcement is conducting training on Alcohol Screening and Brief Intervention (aSBI) and CHOICES and provide technical assistance on implementing the interventions into clinic settings have been identified as barriers for promoting successful adoption and integration into primary care clinics as a standard of care. This program announcement addresses these barriers by focusing on building capacity with the awardee organization(s) to provide training and technical assistance to clinics serving AI/AN populations. Street Outreach Program Grant

This will fund grantees to provide street-based services to runaway, homeless, and street youth who have been subjected to, or are at risk of being subjected to, sexual abuse, prostitution, or sexual exploitation.
